REGISTRATION
 

Here is the link to the Registration Form. This form is a Microsoft Excel file. If you do not have access to Excel, email your address and fax number to Norris@amnh.org and you will be sent a hard copy.

We are offering two categories of registration. General Registration covers all events from 11 to 15 May, with the exception of the conference banquet, the roundtable lunch on 12 May, and the box lunch for the special interest groups on 15 May. We are also offering Day Registration for the special technical session on 14 May. You should select only one of these two options.

SPNHC Members can a substantial discount when registering for the meeting, which is yet another reason to consider the benefits of SPNHC membership. To join SPNHC, go the Society's website at http://www.spnhc.org

When you have completed the form, print it off and mail it, along with a check or money order, to the address shown at the bottom of the form. The deadline for the preferential registration rate is 15 March.

Places for the Yale trip, conference banquet, and workshop are limited, so register early. In the event that you are unable to get a place on the Yale trip, you will be offered the opportunity to attend the session on database design for tissue repositories. There is no additional charge for this.

Visa and MasterCard payments can also be accepted, but are subject to a 3 percent surcharge. If you are paying be credit card, you should print off and sign the form before mailing it in. Do not send the form by email.
